Form of Notice.

A notice—

1. (a) under sub-section (1) of section 61 shall be in accordance with the form in schedule ‘B’ appended hereto;
2. (b) under proviso to sub-section (1) of section 61 shall be in accordance with form in schedule ‘C’ appended hereto;
3. (c) under sub-section (1) of section 62 shall be in accordance with form in schedule ‘D’ appended hereto;
4. (d) under sub-section (2) of section 62 shall be in accordance with form in schedule ‘E’ appended hereto; and
5. (e) under sub-section (3) of section 62 shall be in accordance with form in schedule ‘F’ appended hereto.
