Provided that a private health establishment in existence at the commencement of this Act shall apply for registration within such period from such commencement as may be prescribed.

1. (1) Every application for registration shall be made to the prescribed authority in such form and accompanied by such fee as may be prescribed.
2. (2) Every application for registration shall contain such particulars as may be prescribed.
