Information as to change of Address of Clinical Establishment

Any change in the address or situation of, or of staff belonging to, a clinical establishment shall be communicated by the owner, proprietor or management, thereof to the supervising authority "not later than fifteen days" after such change with specific mention as to the exact date when such change occurred.
