Form and verification of Statements under Section 58 & 60.

The statements submitted to the Registrar under Sections 58 and 60 of the Act shall be in Forms I & II respectively, annexed to these rules and shall be verified in the manner sect forth in the manner set forth in the note at the bottom of these forms.
